Proof of residential address requires two of the following reflecting your name and address: deed, mortgage statement, lease agreement, voter registration, vehicle registration, W2 form, 1099 form, or a homeowners, automobile, or life insurance policy.
If you have recently changed your name, update your records with the Social Security Administration before you apply for your Florida Driver License. The name assigned to the social security number must match the name that will appear on your Florida Driver License.
Hospital birth certificates cannot be accepted.
A birth certificate must be issued by a government agency.
